The primary purpose of the Dietary Cook position is to prepare food for the residents at the facility.

JOB DUTIES & RESPONSIBILITIES:

Administrative Functions

· Review menus prior to food preparation.

· Inspect special diet trays to assure that the correct diet is served to the resident.

· Complete necessary forms, reports, etc. as requested by the Dietary Manager or Dietitian.

· Coordinate food preparation services with other department as necessary.

· Work with the facility’s dietitian as necessary and implement recommended changes as required.

· Ensure that all dietary procedures are followed according to facility policies.

· Ensure that menus are maintained and filed according to facility policies and procedures.

· Assist in establishing food service production lines, etc. to assure that meals are prepared on time.

· Process diet changes and new diets as received from Nursing Services.

Dietary Service Functions

· Prepare meals according to planned menus.

· Prepare and serve meals that are tasty and appetizing in appearance.

· Serve food in accordance with established portion control procedures.

· Prepare food for therapeutic diets according to planned menus.

· Prepare food according to standardized recipes and special diet orders.

· Prepare and serve nutritional supplements and snacks.

· Be sure that appropriate equipment and utensils are provided with the resident’s meal tray.

· Review care plans to assure that current dietary information is consistent with food served.

· Prepare and serve substitute foods to residents who refuse foods served.

· Make only authorized food substitutions.

· Assist in food preparation for special meals, parties, etc.

Personnel Functions

· Develop and maintain a good working relationship with other food service personnel as well as with other department associates so that food services can be properly maintained to meet the resident’s needs.

· Create and maintain an atmosphere of warmth, personal interest, and positive emphasis as well as a calm environment throughout the department.

Safety and Sanitation Functions

· Prepare food according to sanitary regulations as well as facility policy.

· Ensure that safety regulations and precautions are followed at all times by all personnel.

· Follow established Infection Control and Universal Precautions procedures when performing daily tasks.

· Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals to the Dietary Manager.

· Assist/direct daily or scheduled cleaning duties.

· Ensure that the department is maintained in a clean and safe manner by assuring that necessary equipment and supplies are maintained in good working order.

· Report all hazardous conditions/equipment to the Dietary Manager immediately.

· Dispose of food and waste according to facility policies.

The primary purpose of the Licensed Practical Nurse position is to provide direct nursing care to residents as a member of an interdisciplinary team of care providers and to plan, supervise and evaluate the day-to-day nursing activities performed by nursing assistants.

Activities Assistant/ Transport Part Time

JOB DUTIES & RESPONSIBILITIES:

Administrative Functions

· Assist in planning, organizing and implementing group and individual activities based on residents’ individual needs as identified in residents’ health care plans.

· Assist in community planning related to the interests of the facility and the services and needs of the resident and family as directed by the Activity Director.

· Assist in maintaining a balance of recreational activities based on the particular demographics of the residents.

· Assist in coordination of activity schedules with other departments as necessary.

· Communicate residents’ programs to residents, residents’ families, volunteers, and facility staff.

· Decorates monthly bulletin boards indicating future activities, holiday themes, and/or picture review of past activities.

· Assists in transportation of residents within the facility from one area of activity to another.

· Participate in group activities to ensure maximum involvement by interested residents.

· Help read incoming mail, and address and post outgoing mail upon the resident’s request.

· Arrange transportation for field trips as requested.

Miscellaneous Activity Functions

· Assist in making appointments for the residents as requested.

· Assist in scheduling movies, planning parties, and providing games/activities for residents.

· Encourage residents to participate in hobbies, crafts and other activities. Provide materials as necessary.

· Develop and maintain a good rapport with all services involved with the care plan to ensure that a team effort is achieved in developing a comprehensive plan of care.

Committee Functions

· Serve on, participate in, and/or attend various committees of the facility as requested by the Activity Director.

· Provide written and/or oral reports of the programs and activities as required by the committee.

· Assist the Activity Director in evaluating and implementing recommendations from established committees as they may relate to activity services.

· Document status of each resident on the chart at least weekly, more often if significant change occurs or as directed. Follow up with monthly and quarterly summaries as instructed.

· Make rounds with physician, as directed, and record visits in resident’s chart.

· Transcribe, accurately and promptly, all physician’s orders.

· Direct documentation and make weekly detailed evaluation of all resident documentation so that chart reflects progress and condition of resident at all times.

· Approve and sign charts when observations are made.

· Insure that all medications and treatments are charted and signed following administration of the medication/treatment, by the administering person.

· Report all discrepancies noted concerning physician’s orders, diet change, charting error, etc. to the ADON or DON.

· Complete accurate accident/incident reports as necessary.

· Fill out and complete transfer forms in accordance with established procedures.

Drug Administration Functions

· Prepare and administer medications as ordered by physician.

· Verify the identity of the resident before administering the medication/treatment.

· Ensure that prescribed medication for one resident is not administered to another.

· Ensure that an adequate supply of floor stock medications, supplies, and equipment are on hand to meet the nursing needs of the residents.

· Order prescribed medications, supplies, and equipment as necessary and in accordance with established policies.

· Ensure that narcotic records are accurate for your shift.

· Notify the ADON or DON of all drug and narcotic discrepancies noted on your shift.

· Prepare, administer and record all parenteral, intramuscular, sub-cutaneous injections.

· Administer I.V. fluid therapy in accordance with policy and procedure.

· Administer or supervise all treatments prescribed by physician.
